Patient Resource Coordinator

• Assists in creating positive outcomes for patients and families by helping them navigate healthcare challenges.
• Supports the Social Worker with assessments, assists the clinical care team and Social Worker in designing supportive care plans, and provides education and consultation regarding community resources.
• Provides patients and family members with information to adhere to the health‑care treatment plan and promote overall wellbeing.
• Arranges internal support groups and related functions.
• Serves as a liaison between the health‑care system and the community.

• Associate's degree or two years of college/university credit.
• Bachelor's degree preferred.
• At least three years of work experience.
• Health‑care experience preferred.
