Team Leader (RN) – Home Health Care – Full Time – OFP

Under minimal supervision, is responsible for case load/referral management and the overall activities of a specific Henry Ford at Home team. Utilizes the knowledge and application of nursing principles to ensure safe and effective coordination of patient care in their place of residence. Performs duties of the Home Health Care nurse.

• Requires graduation from an accredited Registered Nurse program.
• Current licensure by the State of Michigan as a registered nurse is required.
• Requires at least one (1) year of progressively responsible home health care nursing experience.
• Knowledge of computer use is required to electronically document patient care.
• Strong interpersonal and customer services skills required in order to interact effectively with individuals from various cultural and socio-economic backgrounds.
• Must possess a current State of Michigan driver's license, an acceptable driving record, and reliable transportation.
• Must be able to lift, carry, transport and move clients, if needed.
