About The Role:

Longevity Health is looking for a Nurse Practitioner or Physician Assistant with a passion for working with seniors in long‑term care settings who are prepared to travel and float throughout the state of Michigan. In this role, you'll be responsible for a panel of enrolled members receiving nursing home care. You'll be supported by a dedicated clinical leadership team that provides education and guidance on best practices for effectively caring for our members.

As part of your role, you'll conduct comprehensive assessments and develop individualized care plans for each member. You'll actively participate in nursing facility care team meetings, maintain frequent communication with members' families and loved ones, and ensure the care team stays aligned with members' goals and needs.

Key Tasks and Responsibilities
  • Provides collaborative primary care and services to a panel of Longevity Health Services including treating in the most appropriate setting based on the member’s goals of care.
  • Executes/delivers contracted plan’s Plan Model of Care activities, treating in the most appropriate setting, develops and implements advanced care planning, embraces and executes treating in place model.
  • Facilitates, administers Health Plan clinical operations functions—prior authorization, Part A and B services, Medication Reviews, HEDIS and STARs quality gap closure, promoting membership growth.
  • Serves as a trusted advisor to members, loved ones and nursing home staff and member advocate, educates care teams on member needs and early identification of changes in condition, notifying APP and telehealth partners for member changes.
  • Delivers effective and frequent communication and coordination with members’ PCPs, loved ones and care team members on members’ care needs, goals of care, treatment plans/changes.
  • Supports overall market goals/targets and balances member caseloads’ demands with other team member support needs.
  • Promotes Longevity Health with members, loved ones and nursing facilities and other stakeholders.
  • Can function autonomously and is accountable for practice.
  • Establishes meaningful relationships and can function in the nursing home environment.
  • Is member‑centric and customer service oriented, establishing meaningful relationships that ensure effective and high‑quality care for Longevity Health members.
  • Performs other related duties as assigned.
Supervisory Responsibilities
  • There are no supervisory responsibilities for this position.
Credentials & Coverage
  • State unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner. Will allow acute care nurse practitioner with nursing home/geriatric or hospice palliative experience OR state unrestricted licensed Physician’s Assistant NCCPA board certified with nursing home/geriatric or hospice/palliative experience.
  • Must be willing to travel to multiple facilities throughout the state.
  • Active DEA License registered in the state of practice is required.
  • Must have an active national provider number (NPI) and an active Medicare provider number.
  • Must be eligible to participate in the Medicare program as a provider and registered.
  • Valid state driver’s license with a good driving record and proof of automobile insurance required.
  • Auto insurance coverage as required by your state of residence. New employees will be responsible for providing proof of coverage prior to hire and on an ongoing basis.
  • Must be present in SNF communities/buildings during business hours daily.
