GENERAL SUMMARY
The Nurse Practitioner or Physician Assistant serves as a first-line provider within a collaborative, multidisciplinary team, delivering inpatient and outpatient care to advanced heart failure and LVAD patients. Responsibilities include management of the Advanced Heart Failure (AHF) clinic, inpatient rounding with the AHF team, consultation for LVAD readmissions, medication management, device interrogations, and urgent clinic visits for VAD patients. The APP provides comprehensive patient and staff education, establishes and coordinates outpatient follow-up plans, and performs clinical assessments, diagnoses, and treatments consistent with credentialing, training, and experience.
The role requires close collaboration with attending physicians, advanced practice providers, nurses, consultants, and other members of the care team to ensure safe, efficient, and high-quality patient outcomes.
- Master's degree in Nursing (for Nurse Practitioner) or completion of an accredited Physician Assistant program.
- Previous Advanced Practice Provider experience preferred.
- Required experience in the care of cardiac patients.
- Cardiology experience preferred.
- Experience with ventricular assist devices (VADs) preferred but not required.
- Current certification as a Certified Nurse Practitioner or Physician Assistant.
- Current state licensure as a Registered Nurse (NP) or Physician Assistant, as applicable.
- Current Basic Life Support (BLS) certification.
- Advanced Cardiac Life Support (ACLS), Advanced Trauma Life Support (ATLS), and/or Pediatric Advanced Life Support (PALS) as required by the department.
