Licensed Practical Nurse (LPN) or Registered Nurse (RN) Full Time Day Shift
Eventide Heartland Care Center is currently seeking an LPN or RN to work full time (72 hours/pay period) on our Day shift (6 am
-6:30 pm). This position’s schedule includes every other weekend and holiday rotation. $2 an hour weekend shift differential added to base wage!
- Assess residents’ needs and health status through physical assessments, observations, and interviews on a continuing basis.
- Provide individual attention, encouraging each resident’s ability to maintain or attain the highest physical, mental, and psychosocial well‑being.
- Observe and follow the residents’ plans of care; update and make appropriate changes in plans of care when necessary (interdisciplinary care plans, physician orders, and CNA/NA‑R care plans).
- Administer medications and treatments and monitor both for effectiveness.
- Provide direct nursing care to residents whenever necessary and according to established nursing policies, procedures, and standards.
- Comply with rules and regulations set forth by the Minnesota or North Dakota Department of Health and the Center for Medicare/Medicaid Services.
- Provide accurate documentation in the residents’ electronic medical record according to established nursing policies, procedures, and standards.
- Assist in the orientation and training of new LPN team members.
- Report resident needs, problems, and observations regarding residents’ conditions to the Charge Nurse and/or attending physician.
- Ensure adequate staffing levels for a scheduled shift and correct staffing deficiencies appropriately.
- Monitor the activities and functions of subordinate staff, which may include participating in the disciplinary process.
- Ensure that work responsibilities of subordinate staff are completed thoroughly and accurately within scheduled shifts and according to established resident care policies, procedures, and standards.
- Provide information, education, and support for assigned residents and their families.
- Communicate the needs of residents to resident care staff in the unit, as appropriate.
- Interact with physicians and other health care facilities as needed, according to the needs of the residents.
- Continually update nursing skills and knowledge by attending in‑services, continuing education programs, and staff meetings.
- Coordinate and participate in resident care conferences and communicate information effectively to families.
- Assume responsibility for promptly reporting any incidents or evidence of abuse, neglect, or violation of residents’ rights per policy.
- Must be accountable to manage the electronic timecard on a daily basis.
- In accordance with the team philosophy and Eventide’s commitment for all staff to provide quality resident care, the person filling this position may be occasionally required to carry out or assist with other tasks in addition to the duties listed on this job description.
- Must have an active ND LPN or RN license or be able to obtain upon hire.
- Experience in Long Term Care preferred, but not required.
