Sanctuary Personnel is seeking an experienced Band 6 Community Psychiatric Nurse (CPN) to join a Dementia Service on a locum basis.

You will provide high-quality care to adults with dementia and related cognitive conditions, supporting assessment, care planning, risk management, and multi-disciplinary collaboration. The role involves community-based work, so confidence working independently and managing a caseload is essential.

• Conduct comprehensive assessments and review care plans
• Deliver specialist nursing interventions to individuals with dementia
• Manage and prioritise a community caseload
• Work collaboratively with MDT members, carers, and external services
• Monitor mental and physical wellbeing and respond to clinical risks
• Maintain accurate records and documentation

• NMC registration as a Mental Health Nurse
• Experience working within dementia or older adult mental health services
• Strong risk assessment, communication, and caseload-management skills
• Ability to work autonomously in the community
