Transition Nurse, Band 6 - Children & Young
Julia's House in Devizes is seeking a Transition Nurse (Band
6) to support children and young people with life-limiting conditions. You will manage a caseload, act as Named Nurse, and coordinate care plans centred on the individual needs and wishes of each family.
You will guide transition from children’s services into adulthood, work with schools, hospitals, and adult services, and promote advocacy, best interests decisions, and safeguarding.
