Job SummaryThe Physical Therapist (PT) provides high‑quality, patient‑centered care by managing the complete process of therapeutic care for an assigned group of patients in coordination with the physician and the patient care team. The PT completes patient assessments, develops and implements individualized care plans focused on goal achievement and positive patient experience, and provides or directs a majority of the patient’s physical therapy throughout the continuum of care.
Responsibilities- Provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
- Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
- Provide purposeful and skilled therapeutic care at the patient’s home, using procedures and techniques to produce a change in condition/function consistent with the physician’s plan of care.
- Instruct patient, family and caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
- Prioritize caseload, determine daily staffing needs and coordinate assignments/handoffs. /ul>Qualifications
- Licensed Physical Therapist
- Master’s Degree
- Basic Cardiac Life Support (BCLS) or BCLS Instructor certification obtained within 30 days of employment start date.
