We are seeking an experienced pharmacist, to work as part of a multidisciplinary team at the Practice providing patient-facing dispensing, advice, treatment and administrative support alongside a wider team of healthcare professionals.
You will provide clinical leadership for medicines optimisation and be responsible for managing the prescription process and queries, undertaking clinical medical reviews whilst supporting and advising patients with chronic disease management issues.
This includes helping to manage long-term conditions, advising those taking multiple medicines (polypharmacy) and delivering clinical advice about treatments. You will also assist with communication across a patient's care pathway, manage medicine shortages by suggesting suitable alternatives where appropriate and mentor newer pharmacists.
Main duties of the job- Providing clinical expertise & being an educational resource for colleagues in relation to medicines e.g. dosage, side-effects, alternatives whilst helping to address the public health & social needs of patients within NICE & other guidelines
- Liaising with & proactively developing relationships with an MDT team (including colleagues, NHS professionals & private organisations e.g. CCGs) & act as the main point of contact for medicines
- Manage the process of medicine reconciliation on admittance/discharge from hospital, liaise with patients & community pharmacists to ensure patients have the medicines required
- Managing, auditing & reviewing prescription/repeat prescription policies to improve the quality, safety & cost-effectiveness of prescribing
- Proactively seek to reduce medicine-related admissions & readmissions by identifying medicine-related issues & supporting patients to get the best outcomes from their medicines.
- Handling patient & healthcare professionals prescription queries, providing remote support where needed
- Identify patients that require med reviews including patients clinical condition, monitoring & care arrangements
- Perform regular reviews to identify & address any medicine-related issues, ensure patients get the best outcome from medicines prescribed
- Make appropriate suggestions to GP & other clinicians for patients ongoing treatment & management of referrals
- Ensure repeat reauthorisation processes are adhered to for high-risk medicines
About the Practice:
Parish Fields is a well-established, friendly GP Practice located in the market town of Diss on the Norfolk-Suffolk border.
We have approximately 8,200 registered patients and an onsite modern and well‑run dispensary which dispenses to approximately 30% of these patients.
The Practice utilises the Systm
ONE clinical system.
There is free clinical staff car parking available on site.
