• Provide safe, compassionate nursing care to residents based on individualized care plans
• Administer medications and treatments accurately
• Monitor residents’ health and report changes to the RN, provider, and families as needed
• Document assessments, care, and observations in the EHR
• Support and guide caregivers and CNAs during daily care
• Communicate effectively with physicians, families, and the care team
• Participate in care plan meetings, quality initiatives, and state surveys
• Follow infection control and safety procedures
• Complete other duties as assigned

• High school diploma or GED
• Active, LPN license in the state of Illinois
• Supervisory experience preferred
• Experience in long-term care or a similar setting preferred
• Compassionate, dependable, and strong communication skills
• Ability to maintain resident confidentiality
• Ability to lift, move, and assist residents safely
• Must pass a criminal background check and required health screenings (TB and others)
• Ability to read, write, and understand English
