Arrowhead West, Inc. is seeking a full-time Case Manager to empower individuals to live meaningful, self-directed lives. You will lead person-centered planning, help clients set goals, and coordinate services across Day, Residential programs, Medicaid/MCOs, SSI/SSDI, SNAP, VR, and health supports.
The role emphasizes advocacy, independence, and strong documentation. A bachelor’s degree with relevant IDD experience is preferred, along with a valid driver’s license.
