Overview

Crisis Practitioner role at NHS – Doncaster Crisis Team. 37.5 hours per week. 24-hour service. This is a permanent Band 6 practitioner position in unplanned mental health services in Doncaster.

The role involves triage assessment and intervention for service users in crisis and will include telephone contact, home visits and assessments in A&E. We are seeking dynamic and positive practitioners to support this new service and deliver a caring and safe service within the community.

The post requires individuals to work alongside partner organisations, third sector and peer support services, with transparency and openness as vital. There are opportunities to contribute to and lead changes as the community working age model develops.

Job responsibilities
  • Carry out triage assessment and intervention for service users in crisis
  • Conduct telephone contacts, home visits and A&E assessments
  • Work with families to support understanding of crisis management
  • Collaborate with partner organisations, third sector and peer support services
  • Contribute to service development within the Crisis Service and community context
Person Specification

Essential

  • Registered Mental Health Nurse with current professional registration
  • Evidence of continued professional development
  • Risk assessment and risk management training
  • Safeguarding Level 3
  • Mentorship qualification

Experience

  • Extensive experience in mental health settings
  • Experience of case management for people presenting in mental health crisis
  • Leading in risk assessment and risk management planning
  • Co-producing care and crisis plans with clients and families
  • Multi-agency working and cross-service collaboration
  • Involving service users and carers in care delivery

Desirable

  • Therapies skills training/qualification
  • Psychosocial interventions
  • Suicide prevention training
  • STORM and supervision training
Additional information

Pay scheme:
Agenda for Change, Band 6 – £38,682 to £46,580 per annum. Permanent contract. Full-time, flexible working.
