As a Specialist Community Public Health Nurse (SCPHN) you will work as part of an integrated team contributing to family health needs assessment. The post holder will lead the 0-5 Healthy Child Programme and work in partnership with the family, education and other agencies to develop, implement and evaluate packages of care. Our service delivery model reflects the National 4, 5, 6 model with a focus on the 6 High Impact areas.

We deliver services in pathways at Community, Universal/Universal Plus and Universal Partnership Plus levels. This is underpinned by a Single Point of Contact (SPOC) which supports our practice by responding to unplanned care enabling the Universal team to focus on providing quality core Health Visiting care. Successful applicants will be allocated to work in one of 4 areas – North, South, East or Central area of Doncaster.

Qualifications
  • SCPHN – Health Visitor NMC Registered Practitioners who are proactive and enthusiastic with excellent communication, interpersonal and organisational skills and are keen to work in Doncaster.

We pride ourselves in leading the way with care and offering our Health Visiting Team an enhanced training package of tools to support their exemplary practice.

Health Visitors have the opportunity to complete evidence‑based training including Brazlton (New Born Observation), Promotional Guides and the Solihull Approach is embedded into our everyday practice.
