Community Learning Disability Nurse Job overview
The North Doncaster Community Learning Disability Team is a specialist Health Care team working to meet the health care needs of adults with learning disabilities.
We are looking to recruit a motivated and enthusiastic practitioner into this challenging and rewarding post.
You will be responsible for assessing and supporting the health needs of adults with a learning disability under the supervision and guidance of the Senior Community Nurse.
Main duties of the job- Responsible for a defined caseload of people requiring support from the service.
- Completing a variety of assessments to determine input required and implementing interventions based on the outcome of these.
- Monitoring and reviewing the effectiveness of interventions and liaising with other members of the multi-disciplinary team.
- Working with individuals, families, other professionals and carers to ensure a holistic approach.
- Ability to work as part of an established team and also have the skill and confidence to work autonomously when required.
- Registered Nurse – Learning Disabilities.
- Evidence of consistent training relating to evidence-based care.
- Relevant statutory training.
- Continuing Health care assessments.
- Understanding the importance and evidence of commitment to continuing self-development and training within a social care or health service delivery setting.
- Experience in a variety of care and support settings for people with learning disabilities.
- Experience of care planning for complex health care needs.
- Knowledge of current legislation associated with community care.
- Knowledge of Positive Behavioural Support.
- Experience of coordinating services for people with learning disabilities.
- Experience of working within a community team.
- Experience of working with family carers.
- Within allocated caseload ability to prioritise workload and manage time effectively.
- Ability to provide professional support to the senior nurse and be able to manage caseload in the senior nurse's absence.
- Understanding the role and responsibility of the post to all involved organisations and agencies.
- Ability to set, monitor and audit standard of care and service quality.
- Clear knowledge and understanding of the statutory policy frameworks relating to services for people with a learning disability, including Mental Health Act 1983, Mental Health Act 2005, Mental Capacity Act 2005.
- Safeguarding risk assessment and management.
- Makaton training.
