Senior Physiotherapist

We are looking for a motivated and enthusiastic Physiotherapist to join the Home First Service within Doncaster. The role is based across Community Physiotherapy, Urgent Community Response and Enhanced Care Home Team, with possible cover of Home First Rehabilitation Service. The post is a Band 6, 37.5 hours per week with flexible working and bank shifts available.

The Home First Service provides assessment, treatment and care support to patients in bed‑based units or at home/ care homes, aiming to avoid hospital admissions and support early discharge. The post holder will support all areas within the Home First Rehabilitation pathways.

The role requires HCPC registration, post‑qualification experience in rehabilitation, ability to drive/travel across Doncaster area, and excellent communication skills. The successful candidate will work autonomously and support the multidisciplinary team in developing holistic management plans.

Main duties of the job
  • Provide specialist assessment, diagnosis and treatment of patients with complex physiotherapy problems as an autonomous practitioner within clinical speciality area.
  • Take line management responsibility for staff within the service area to provide a responsive and effective service.
  • Support the Team Leader in operational management and service developments on a local or service‑wide level.
  • Provide clinical education and training.
  • Take responsibility for specific projects or initiatives contributing to the overall delivery of the service within a clinical speciality area.
  • Provide information to the Senior Management Team.
Qualifications
  • Diploma/Degree in Physiotherapy
  • Registration HCPC
Experience
  • Post‑graduate experience as a Band 5 physiotherapist in a broad range of areas, including relevant experience in the specific clinical specialty.
  • Evidence of relevant CPD and community working.
  • Experience working with older adults, supervising junior/support staff, and NHS working.
  • Understanding of clinical governance, professional conduct and confidentiality.
  • Extensive experience working as part of a physiotherapy and multidisciplinary team (MDT).
  • Experience in personal development appraisal, research, audit, reflective practice and outcome measures.
  • Experience with patients having falls, long‑term and palliative conditions.
  • Experience networking within physiotherapy and the wider Health & Social Care community.
Knowledge
  • Understanding of current NHS issues and initiatives.
  • Awareness of relevant clinical guidelines (e.g., falls guidelines).
  • Clinical knowledge of a broad range of diagnoses and conditions relevant to the post.
  • Understanding of clinical risk management and ability to formulate plans to reduce risk.
  • Knowledge of HCPC standard of practice and professional registration requirements.
Skills
  • Communicate complex information clearly to professionals, patients and families, overcoming barriers to communication.
  • Assessment, clinical reasoning and problem solving for complex case management.
  • Maintains judgement under pressure and demonstrates a positive approach to problem solving.
  • Organisational skills including time management and prioritisation to manage clinical and administrative workload.
  • Confident user of computers and IT databases; proficient in Word, Excel and PowerPoint.
  • Flexible approach to meet needs of patients and service delivery.
  • Recognises own limitations, seeks supervision and implements reflective practice.
  • Understanding of the Home First pathways.
  • Experience of project work at departmental level.
  • Ability to work collaboratively, influence and coach others.
