Due to an internal restructure, an exciting opportunity has arisen for a Clinical Coding and Summarising Administrator to join our established Medical Summarising Team. The successful candidate will be responsible for summarising and accurately coding patient records, including records transferred from previous GP practices, ensuring high-quality clinical data is maintained to support safe patient care. The role involves reviewing medical information, applying appropriate clinical codes, maintaining data accuracy and supporting ongoing data quality initiatives.

The successful candidate will demonstrate a high level of accuracy, excellent organisational skills and a commitment to delivering outstanding service to both patients and colleagues.
Main duties of the jobAs part of our Medical Summarising Team, you will play a key role in ensuring patient records are accurate, up to date and clinically coded correctly to support safe and effective patient care. The role involves reviewing and processing medical information received from hospitals, community services and previous GP practices, ensuring relevant clinical information is recorded appropriately within the clinical system.
You will contribute to maintaining high standards of data quality and help ensure the practice meets contractual and quality requirements. The post-holder will work closely with clinicians and administrative colleagues, using excellent attention to detail and organisational skills to manage workloads effectively while maintaining confidentiality at all times. We are looking for a motivated individual who enjoys working in a fast-paced healthcare environment, is confident using IT systems and is committed to continuous learning and development.
Full training and ongoing support will be provided to help you develop your skills and knowledge within this specialist area of primary care. As the role may require working across practice locations, the successful candidate must be able to travel between sites when required.

Jobdescription Clinical Coding and Summarising
- Summarise newly registered patient records and records received from other healthcare organisations.
- Accurately extract and record relevant medical information into the patient record.
- Apply appropriate clinical codes using SNOMED and other approved coding systems in accordance with practice protocols.
- Ensure diagnoses, procedures, investigations, medications, allergies, immunisations and significant medical history are recorded accurately.
- Review patient records for completeness and identify discrepancies requiring clarification.
- Maintain accurate and clinically relevant problem titles within patient records.
- Ensure coded information supports effective clinical care, disease management and reporting.
- Assist with data cleansing and data quality improvement initiatives.
- Support the maintenance of practice registers and disease-specific databases.
- Identify and escalate concerns regarding clinical information where appropriate.
- Organise and maintain electronic patient records in accordance with practice procedures.
- Scan and attach documentation accurately to patient records.
- Process incoming correspondence and ensure appropriate filing within clinical systems.
- File, store and retrieve records as required.
- Maintain an organised and efficient workflow to ensure timely processing of documents.
- Assist with data validation exercises and coding audits.
- Support the practice in achieving QOF and other contractual requirements through accurate coding.
- Participate in audits and quality improvement projects as directed.
- Review coding accuracy and contribute to continuous improvements in data quality.
- Support the development and implementation of coding policies, procedures and best practice guidance.
- Assist in preparing data for reporting and performance monitoring purposes.
- Provide administrative support to clinical and non-clinical teams when required.
