Nottinghamshire Healthcare NHS Foundation Trust is recruiting a Clinical Pharmacist to undertake structured medication reviews and medicines optimisation for patients with long-term conditions, frailty or high-risk prescribing.
You will support repeat prescribing, medicines reconciliation post-discharge or transfer of care, and provide specialist clinical advice to GPs, nurses and other healthcare professionals, with opportunities to act as an Independent Prescriber where qualified.
