The City of Doncaster Council puts improving the life of its residents at the centre of everything we do. We are looking for a confident and ambitious individual to help deliver our early intervention and prevention offer through the Neighbourhoods and Wellbeing Service.

Duties include:
- Delivery of an all-age, person-centred service in a locality base.
- Support adults by promoting the Care Act Wellbeing Principles including support to improve; physical health, mental health, work, education, training, social and economic wellbeing, relationships, living environment and contribution to society. Always treating people with dignity and respect.
- Early identification and support of unpaid, adult carers.
- Early identification and support of those from the Armed Forces community.
- Support people to lead healthy, connected and active lifestyles, which allow them to remain living independently in their homes and communities for as long as long as possible.
- Ensuring that individuals have choice and control in how their support is provided.
- Customer facing delivery of early intervention and prevention solutions to reduce escalation to costly services.
- Work with a range of partners, residents, services and the community to promote and influence early intervention and prevention, relational practice, improved wellbeing, Community Led Support and community capacity building.
- To develop an understanding of the needs of local people which will enable communities to do more for themselves and develop their own solutions.
