An exciting opportunity has arisen for an experienced and motivated leader to join our Community Nursing Service as a Team Lead. As part of our evolving Neighbourhood Team model, you will lead multidisciplinary community teams to deliver high‑quality, safe and effective care in patients’ homes and local communities, providing visible leadership, operational oversight and professional support to a diverse workforce, ensuring services are responsive, person‑centred and aligned to organisational priorities.

This is an excellent opportunity for an experienced leader passionate about developing people, driving service improvement and supporting teams through change. Working closely with clinical and operational colleagues, you will shape the future of community services while maintaining a strong focus on quality, performance, staff wellbeing and patient experience.

The Team Lead will provide visible operational and professional leadership to nursing teams working within community settings, supporting the delivery of safe, effective and person‑centred care.

• Providing visible leadership and support to staff, creating a positive team culture that promotes wellbeing, engagement and continuous improvement.
• Managing workforce performance, recruitment, attendance, supervision and professional development.
• Overseeing the day‑to‑day delivery of services, ensuring resources are effectively deployed to meet patient and service needs.
• Working collaboratively with clinical, operational and multidisciplinary colleagues to coordinate care and improve outcomes for patients.
• Monitoring quality, performance and patient experience, taking action to address risks, concerns and opportunities for improvement.
• Leading and supporting service developments, quality improvement initiatives and organisational change programmes.
• Ensuring compliance with professional, organisational and regulatory standards, maintaining a strong focus on patient safety and governance.

• Current NMC registration
• RGN
• Education at degree level or evidence of ability to work at this level.
• Qualification in District Nursing or equivalent.

• Experience of leadership.
• Experience in service development.

• Appropriate demonstrable experience as a caseload holder.
• Innovative and dynamic approach.

• Excellent communication skills, tactful and diplomatic.
• Ability to manage under pressure.
• Motivated, self‑reliant and approachable.
• Evidence of ongoing CPD.
