Position Details
Stroke Support Worker (Homebased, Doncaster, South Yorkshire).
Role:
Deliver person‑centred support to stroke survivors on the acute ward and via phone.
Hours:
21–28 hrs per week (minimum 21 hrs, up to 28 hrs), with twice‑weekly on‑site visits of 3 hrs each. Travel to Doncaster Royal Infirmary is required; occasional overnight stays may be required.
Salary: £17,000–£22,600 per annum (based on hours; FTE £28,340.58).
Contract:
12‑month fixed term.

Responsibilities- Deliver person‑centred conversations with stroke survivors on the acute ward.
- Assist survivors in connecting with the Association’s services and exploring further support options.
- Provide phone support to stroke survivors when out of the hospital.
- Co‑operate with the Integrated Offer team to test and implement new approaches to care.
- Adapt session length, timing, and cadence based on hospital workflow and patient needs.
- Attend occasional overnight stays as required.
- Experience communicating directly with stroke survivors, either face‑to‑face or by phone.
- Strong person‑centred support background.
- High emotional intelligence and good judgement, especially in emotionally charged situations.
- Excellent listening skills and clear communication.
- Resilience to handle complex situations in a hospital environment.
- Comfortable with coaching and reflective practice to drive continuous improvement.
- Resident of the UK with right to work.
