Reablement Officer (CDC)

Based at Positive Step Social Care Assessment Unit, Bentley.

Salary:
Grade 5, £25,584 per annum, pro rata plus 8.5% all inclusive allowance for weekend and shift working.

Contract:
Permanent.

Hours:
Part time, 32 hours per week.

Closing date: 02 August 2026.

Enhanced Level DBS Disclosure is required for appointment to this post.

Responsible for supporting service users with daily living needs, including personal care, nutrition, social and emotional support in line with individual care plans; facilitating stimulation and promoting independence; supporting access to activities and leisure during their stay. Implement and review person‑centred support and therapy plans in accordance with unit guidelines; maintain up‑to‑date knowledge of care plans and participate in daily handovers.

Draft clear, factual reports and monitoring forms, reporting any concerns immediately to the duty manager. Respect dignity and choice of service users; assist in domestic duties as required. Adhere to health and safety procedures and work a rota covering 7 days, weekends and bank holidays with flexible shift times between 07:00 and 22:00.

Required skills and attributes include:

• Experience in adult social care or transferable skills from other roles or voluntary work.
• Ability to write clear, factual reports and accurate monitoring documentation.
• Knowledge of health and safety procedures in a care setting.
• Strong verbal communication skills and English fluency under the Immigration Act 2016.
• Willingness to undertake an enhanced Level DBS check.
