The Community Rehabilitation Team (CRT) West supports people living with complex mental health problems to achieve their goals and gain the skills and confidence to live as independently as possible.

CRT West is a multidisciplinary team (MDT) that discusses, formulates, and delivers individualised plans. We are looking for a dynamic and enthusiastic Assessment & Support Coordinator to join the team on a full‑time basis. The role involves working creatively to deliver a high standard of care that optimises patients’ health, wellbeing and recovery. You will provide stability, help them develop meaningful occupation, and manage the symptoms of mental illness.

Key responsibilities:

• Lead on all aspects of social care within the team.
• Work collaboratively in an MDT to formulate and deliver individualised care plans.
• Provide stability and support to enable patients to live independently.
• Ensure application of the Care Act, Mental Capacity Act, and Mental Health Act in all decisions.

Essential qualifications:

• Experience or keen interest in working within a specialist mental health service.
• Knowledge and application of the Care Act, Mental Capacity Act and Mental Health Act.
• Full access to a vehicle to cover the Dorset Council locality.
