About the role

As an Assessment Support Coordinator, you will be working alongside Social Workers, Occupational Therapists, and health colleagues across a range of settings to support people to remain as independent as possible and linked in with their local networks.

  • you will be undertaking strengths-based assessments and support planning, in accordance with the Care Act 2014
  • you will assist individuals to identify their goals and how best to achieve them to support their independence, and where appropriate complete mental capacity assessments
  • you will be working as part of a busy team and alongside other teams within the local authority. Multi-disciplinary practice will be integral to the role

Our teams have a strong track record in supporting new starters to Dorset Council Adult Services by being friendly and supportive, developing individuals and offering a detailed and comprehensive induction plan.

We are recruiting Assessment Support Coordinators to work in the following areas:
  • East Dorset Locality Team, based in Ferndown - full time
  • Purbeck Locality Team, based in Wareham - full time and part time
  • Community Learning Disability Team, based in North Dorset - part time
  • Home First Hospital Discharge Team - full time (12 month fixed term contract only) based in North Dorset
  • Home First Hospital Discharge Team, based in North Dorset - part time

All roles require a minimum of two days a week based in the office. Please note within your application which of these role/s you are particularly interested in.

About You
  • you will have excellent communication skills, enjoy working in a fast-paced environment, have a flexible approach and be able to work closely with a range of professionals
  • you will have experience of working in a social care setting where you have supported people to maximise their potential but applications from candidates with transferable skills is also welcomed
  • you will be confident in meeting new people and be able to adapt to work across a range of social care settings
  • you will be creative and passionate about enabling people to be as independent as possible
  • you will be able to fulfil the travel requirements of the post
