This role has a starting salary of £27,318 per annum (pro rata to £54,636 FTE), based on an 18-hour working week.

This is a secondment opportunity with an end date of31/03/2027.

We are seeking an experienced qualified Social Worker to join our Connect to Community (C2C) East 2 team on an internal secondment as an Assistant Team Manager. This opportunity is open only to registered Social Workers and is ideal for practitioners looking to develop leadership experience while remaining closely connected to frontline social work practice within a fast‑paced, multidisciplinary environment.

This is a hybrid role where the expectation is a minimum of 20% of the week in the office, as well as working in locations as required to meet the demands of the role as required. Your base location will be either Pippbrook House, Dorking or Dakota House, Weybridge.

About Connect to Community (C2C)

Connect to Community (C2C) is the front door for adults requiring further support following initial triage by the Information and Advice Service. It is a strengths‑based, multidisciplinary service designed to deliver timely, proportionate and resident‑focused responses.

C2C brings together professionals from different disciplines to:

  • Hold daily multi‑disciplinary discussions to review new contacts
  • Reduce unnecessary handovers and delays
  • Deliver early social work intervention, advice and decision‑making

The work within C2C is varied and includes:

  • Strengths‑based conversations and short‑term social work interventions
  • Safeguarding triage, including application of the Section 42 three‑part test
  • Mental Capacity Act work
  • Urgent support planning and risk management
  • Close working with reablement, technology‑enabled care, housing and community services

Cases remain within C2C until situations are stabilised or can be transferred directly to an allocated practitioner, supporting continuity and improved outcomes for residents.

About the Role

This Assistant Team Manager post is based within Connect to Community (C2C) and combines practice leadership, professional supervision and operational oversight with an ongoing expectation of social work practice and professional decision‑making.

As an Assistant Team Manager, you will provide professional social work leadership within the C2C multidisciplinary team, supporting and modelling high‑quality social work practice, including work with complex and high‑risk cases. You will support staff with complex decision‑making, risk management and safeguarding, leading and contributing to safeguarding decision‑making at the front door.

Your role will involve undertaking and supporting social work assessments and interventions, including complex and urgent work, while promoting strengths‑based, proportionate and legally sound practice. You will offer reflective supervision, professional guidance and line management to practitioners, supporting practice quality and ongoing development, and you will work collaboratively with partners across health, housing, reablement and the voluntary sector to achieve…
