About the roleAn exciting opportunity has arisen for an experienced Band 6 Community Psychiatric Nurse (CPN) to join the STEP Team
. This permanent position offers the chance to deliver high-quality, recovery-focused mental health care to adults with complex needs within a community setting.
You will work autonomously while being fully supported by a skilled and multidisciplinary team, contributing to meaningful outcomes for service users through evidence-based practice, care coordination, and therapeutic engagement.

- Manage a caseload of adults with complex mental health needs
, providing autonomous and skilled nursing care. - Undertake comprehensive mental health and risk assessments
, developing person-centred care plans. - Implement and review care plans in line with CPA and recovery-focused models
, supporting individuals in the community. - Deliver evidence-based interventions, including medication management, psychoeducation, and therapeutic support
. - Monitor mental state and physical health, identifying and escalating concerns appropriately.
- Act as Care Coordinator
, advocating for service users and promoting their independence and recovery. - Assess and manage risks related to self-harm, suicide, and safeguarding
, in line with policy and best practice. - Work within the Mental Health Act and contribute to safeguarding processes.
- Collaborate closely with the multidisciplinary team
, including psychiatrists, psychologists, social workers, and partner agencies. - Support transitions from higher-intensity services to community-based care.
- Provide supervision and guidance to Band 5 nurses, students, and junior staff
. - Contribute to service development, quality improvement, and clinical governance
. - Maintain accurate, timely, and professional clinical documentation.
