About the role;

Are you a Qualified Social Worker with experience in adult mental health and Community Mental Health Teams (CMHT) ?

We have an excellent opportunity for a dedicated Social Worker to join an established multidisciplinary team in Dorset
, supporting adults with complex mental health needs. You will be responsible for a varied caseload, undertaking statutory Care Act duties
, assessments and reviews, while working collaboratively with health and social care professionals.

The role will also involve regular community and home visits across Dorset
, so candidates must be comfortable working within the community.

Key Responsibilities:

Complete strengths-based assessments in linewith the Care Act 2014.
Support safeand timely hospital discharges and prevent unnecessary admissions.
Develop and review person-centred careand support plans.
Work closely with multidisciplinary teams, including NHS colleague sand community services.
Undertake Mental Capacity Assessments and Best Interest decisions where appropriate.
Manage a varied caseload while maintainingaccurateand up-to-date case records.
Promote independence andenable service usersto achieve thebest possible

Requirements:

Qualified Social Worker with current Social Work England registration
.
Previous experience within Community Mental Health, Adult Social Care or CMHT services
.
Strong working knowledge of the Care Act, Mental Health Act and Mental Capacity Act
.
Proven experience managing complex cases, safeguarding and risk
.
Excellent assessment, report-writing and communication skills.
Ability to work effectively within a multidisciplinary team
.
Ability and willingness to undertake community and home visits
.
A confident, professional and person-centred approach to supporting adults with complex needs.
