Overview

Palliative Care Nurse Practitioner to join an expanding Inpatient Palliative Care (Hospice) team.

• 100% inpatient rounding on hospice patients.
• Monday - Friday.
• Support 6 patients per day.
• 5 day workweek, 8am - 5pm, rotating on-call. Practice autonomously.
• Assess, plan, implement, and evaluate care needs.
• Symptom management, chronic disease management, goals of care, advanced care planning.
• EPIC EMR.
• Join a non-profit tertiary referral center with 350 physicians.
• 2 hospitals with 420-bed facility.
• Level II Trauma center.
• DaVinci robotics.
• Academic affiliation of Osteopathic Medicine.
• Inpatient experience in Palliative Care or Hospitalist role required.
