We are looking for a Case Manager to join our team and provide essential non-clinical services that connect clients with medical, social, and educational resources. In this role, you will be a vital link in their journey toward recovery and independence.
What You'll Do- Provide client-centered support:
Help individuals navigate complex service systems and connect them with community resources that meet their specific needs. - Manage and coordinate care:
Develop and monitor service plans, ensuring clients receive the right support and make progress toward their goals. - Build and maintain a resource network:
Continuously research and stay up-to-date on available resources in the community to best serve clients, their families, and our partners. - Document and monitor progress:
Maintain accurate and timely documentation of client progress in our Electronic Health Record (EHR) system. - Collaborate with the team:
Work closely with the Clinical Director, therapists, and other staff to ensure seamless service delivery and address any client needs or conflicts.
- A professional with a strong understanding of case management activities, including linkage, referral, advocacy, and crisis intervention.
- Someone with excellent organizational and time-management skills who can effectively plan and prioritize a flexible schedule.
- An effective communicator with a proven ability to build positive working relationships with clients, families, and community partners.
- A resourceful problem-solver who can identify and connect clients with the services they need.
- Bachelor’s Degree in Human Services field of study (required)
- 2 years experience in Medical and/or Behavioral Health (preferred)
- High degree of organizational skills and attention to detail
- Successful completion of HCBH pre-employment screening and background check
- Has the ability to communicate effectively orally and in writing
- Has a valid driver’s license and eligible for HCBH insurance coverage(s)
- Proficient in Microsoft Office:
Word, PowerPoint, Excel, Outlook - Ability to conceptualize, develop and implement programs to their completion in a fast-paced environment
