The Advanced Practitioner at Monogram Health is a key member of an integrated Care Team that includes a Registered Nurse and a Social Worker. Our Nurse Practitioners and Physician Assistants help patients improve their quality of life in the home and slow the progression of kidney disease, enabling positive health outcomes.

Impact

Using your skills, you will deliver personalized compassionate medical care to individuals mainly with CKD and/or ESRD. You will also be responsible for caring for patients, maintaining accurate records, scheduling, and administering follow‑up appointments as required.

Roles And Responsibilities
  • Conduct comprehensive annual wellness exams on patients in their home or virtually
  • Counsel and educate patients and families about benefits and programs available to help them live healthier lives
  • Document items such as chief complaint, diagnosis, history, review of systems, medications, allergies, assessment, and plan
  • Coordinate care with primary care providers, specialists, and appropriate ancillary services
  • Complete all documentation and paperwork in a timely manner
  • Maintain quality of care standards as defined by the practice
  • Deliver evidence‑based, timely care to reduce avoidable hospitalizations and maximize quality of life
  • Prescribe medications, order tests, and collaborate with patient’s Monogram physician
  • Facilitate closing gaps in care by educating patients about preventive monitoring and scheduling diagnostic testing
  • Assist patients in enrolling to access educational videos
  • Participate in integrated care team meetings
  • Develop and implement treatment plans; assess patient health status; follow up and evaluate patient status
  • Other duties as assigned
Position Requirements
  • Active and unrestricted RN, NP or PA license
  • Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
  • Current and unrestricted DEA certificate
  • BLS certification required (supported through onboarding)
  • 2+ years of direct patient care experience
  • Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home
  • Managed Care/IPA/Health Plan experience preferred
  • Sound understanding of federal and state regulations including HIPAA and OSHA
  • Bilingual (English/Spanish) highly preferred
  • Access to transportation, valid driver’s license and insurance
  • Ability to work independently without direct supervision
  • Proficiency with medical instruments and technology; knowledge of EMR systems
  • Effective communication skills with patients, families, physicians and community partners
  • Proficient in computer‑based data management and point‑of‑interview technology
  • Knowledge of disease diagnosis and prevention; ability to develop treatment plans
