Licensed Practical Nurse (LPN) – Group Home

Schedule:

8:00 AM – 3:30 PM, rotating days with every other weekend

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide compassionate and skilled nursing care for a client residing in a group home setting. The nurse will work alongside Certified Nursing Assistants (CNAs) who provide support with activities of daily living (ADLs).

• Medication administration in accordance with physician orders and facility protocols.
• Comprehensive assessments of client health status, including vital signs, skin integrity, and overall wellness.
• Documentation of nursing care, assessments, and interventions in compliance with regulatory standards.
• Collaboration with CNAs and interdisciplinary team members to ensure continuity of care.
• Health education and support for staff and client as needed.
• Monitoring and reporting changes in condition to supervising RN or physician.

• Current Georgia LPN license in good standing.
• Minimum of 1 year of nursing experience (group home or long-term care preferred).
• Strong skills in medication administration, assessment, and documentation.
• Ability to work collaboratively with CNAs and other care team members.
