Care Coordinator

The Care Coordinator RN (CC RN) is responsible for assessing patients' transitional care needs, coordinating care across the continuum, and engaging with patients and families to ensure comprehensive discharge planning.

Key duties include conducting psychosocial and functional assessments, facilitating interdisciplinary care conferences, and collaborating with physicians and care teams to promote timely care progression.

They manage discharge planning, identify and address barriers to discharge, and facilitate post-acute referrals for a smooth transition.

Documentation of assessments, care plans, and interactions is essential, ensuring records are accurate and up-to-date.

Qualifications include an Associate's or Bachelor's degree in Nursing, RN licensure (single or multi-state), and BLS certification.

Minimum of one year of nursing experience in acute care, with strong communication, organizational, and problem-solving skills.

Must be self-directed, adaptable, and knowledgeable of healthcare regulations and community resources.
