Based at Dover Civic Centre, you will travel across Dover, Sandwich, Aylesham, Ash, Capel-Le-Ferne, Deal and surrounding areas to provide planned daily living support and enable individuals to achieve independence.
The role is customer-facing within a collaborative team including Social Workers and Occupational Therapists.
