- This full-time primary care role will include patient care rounding in a post-acute care setting in a skilled nursing facility (SNF) located in the Dover, Delaware, area
- You will round 5 days a week with the flexibility to choose your rounding schedule
- Primary care rounding in skilled nursing facilities
- Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
- Collaborate with clinicians, facility leaders and others regarding care and services
- Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
- Provide oversight, guidance, and mentorship to our nurse practitioners
- Conduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of care
- Minimum of 1-year primary care experience
- Post-acute care experience a plus
- Active and unrestricted Delaware license or application in process
- Active and unrestricted DEA/prescriptive authority or application in process
- Expected compensation is $210,529 to $270,529 annually with no cap on productivity income potential.
- Sign-on bonus is available in months 1 and 13!
