Population Health Clinical Pharmacy Specialist

The Population Health Pharmacist is a licensed pharmacist who provides clinical pharmacy support to Medicare and Commercial providers. Responsibilities focus on improving pharmacy-related care quality (STAR measures) and cost containment strategies through provider collaboration with value‑based arrangements at Bayhealth.
Responsibilities- Support Bayhealth providers by analyzing reports and EMR data extraction to identify opportunities that improve outcomes based on medication usage (utilizing STAR and HEDIS) and reduce pharmacy costs to health plan members.
- Assess health plan members with pre‑determined medical conditions to identify barriers to care, perform self‑management education, recommend screening, perform MTM services, and develop and implement personalized health improvement plans and goals.
- Collaborate with Bayhealth providers to implement clinical quality data initiatives determined by Bayhealth and participating health plans, maintaining a 5‑STAR rating of 90 % or greater for members annually.
- Participate in various committees, meetings, and audits; develop and maintain policies and procedures that support Bayhealth’s initiatives and meet State and Federal legal requirements and standards.
- Inform providers and health plan members of the health plan’s formulary changes through educational programs annually.
- Assist in identifying health plan members’ needs and intervene to prevent avoidable hospital admissions or readmissions, documenting interventions to assess program success.
- Act as a subject matter expert in pharmacy‑related clinical measures for value‑based care agreements.
- Perform all other duties as assigned within the scope and range of job responsibilities.
- Education:
Bachelor Degree in Pharmacy - Credential(s):
Licensed Pharmacist;
Board of Pharmacy Specialties (BPS) Certified in a specialty with one year of clinical pharmacy experience or Pharm
D with Board Certification in Pharmacotherapy or an advanced degree (e.g., MPH or MBA) - Experience:
One year of hospital pharmacist experience as a clinical pharmacist or completion of an ASHP accredited PGY‑1 residency program.
- Education:
Doctorate Degree in Pharmacy with Board Certification and one year of clinical pharmacy experience or Pharm
D with Board Certification in Pharmacotherapy or an advanced degree (e.g., MPH or MBA) - Experience:
Two years of hospital pharmacist experience as a clinical pharmacist or completion of an ASHP accredited PGY‑2 residency ambulatory program.
