Job Summary
Responsible for the optimization of drug therapy by identifying, resolving, and preventing drug-related problems and improving therapeutic outcomes. Provide patient care in accordance with state and federal pharmacy regulations, and may prescribe medications under collaborative drug therapy management (CDTM) agreements.
Essential Functions- Directs and coordinates pharmacy service activities and delivery.
- Supervises and verifies accuracy of support personnel work during assigned shifts.
- Operates within pharmacy policy and procedures.
- Ensures compliance with target drug programs, formulary management, medication management initiatives, and adverse drug event monitoring.
- Documents and reports clinical interventions, medication errors, and adverse drug events.
- Participates in department quality improvement efforts.
- Follows all applicable policies, procedures, laws, and regulations set by the Board of Pharmacy and other regulatory agencies.
- Doctor of Pharmacy (Pharm.
D) required; no experience in lieu of the degree. - Valid Massachusetts pharmacist license.
- Massachusetts Controlled Substances Registration (MCSR) preferred.
- Minimum of 2-3 years experience as a pharmacist preferred.
- Extensive knowledge in IV admixtures, drug information, ambulatory care, manufacturing, and drug distribution.
- Attention to detail; ability to assess patient data across neonatal to geriatric populations.
- Independent work capability; participation in quality improvement and education of other health professionals.
- Must complete 20 continuing education credits per year to maintain license.
- Familiarity with pharmacy practice settings, legal requirements, prescription calculations, drug identification, manufacturing, packaging, labeling, and drug handling/storage.
