Base pay range: $25.09 /hr – $37.64 /hr

General Summary

The Licensed Practical Nurse (LPN) provides professional nursing care to patients on medical‑surgical units. After competency verification, LPN may deliver care to adult and geriatric populations. Responsibilities include recording vital signs, drawing blood, administering oral and IV medications, performing wound care, assisting with procedures, and maintaining comprehensive documentation.

Responsibilities
  • Read and interpret doctors’ orders, medication names, and lab tests; perform focal assessments for established inpatients.
  • Provide general basic nursing care such as positioning patients, lifting and turning, assisting with bedpan, commode, urinal or bathroom use, and weighings.
  • Provide exceptional customer service that aligns with the Planetree philosophy.
  • Perform routine, uncomplicated treatment procedures including enemas, use of hot and cold compresses, dressing changes, wound cleansing, vital sign collection (orthostatic and rectal), and splinting of extremities; collect and record intake and output.
  • Prepare and administer oral medications as ordered; administer maintenance IV fluids under RN direction; review medication history with patient.
  • Assist with minor medical/surgical procedures in practice settings.
  • Perform venipuncture for blood draws and monitor patient condition before, during, and after venipuncture.
  • Transport patients using wheelchairs or stretchers within the hospital and to/from vehicles; assist those requiring ambulation aid.
  • Identify the need for patient education and advise them on their treatment plan.
  • Recognize abnormal lab results and take appropriate actions.
  • Document treatment records for each patient; immediately report any observed changes in condition or behavior, and document any care given in an acceptable medical‑legal fashion; report unusual incidents promptly.
  • Set up for suturing and procedures using sterile technique; dismantle procedure sets and safely handle soiled instruments.
  • Maintain patient confidentiality; comply with HIPAA and other established compliance guidelines.
  • Ensure compliance with Joint Commission regulations and other safety requirements, including proper disposal of sharps and contaminated materials.
  • Re‑order supplies as required within budgetary limits; maintain workstation cleanliness; restock linen and supplies; manage laundry and cleaning duties.
  • Demonstrate knowledge of current professional practice standards and laws; perform annual self‑assessment and pursue professional development opportunities.
  • Participate in ongoing educational activities such as BLS and NAPPI; maintain professional affiliations; attend scheduled staff meetings; perform tasks assigned by the Charge Nurse/Clinical Coordinator; act as back‑up for other staff members as needed.
  • Contribute to a learning environment by sharing knowledge and providing constructive feedback to peers and other healthcare providers.
  • All other duties as assigned within the scope and range of job responsibilities.
Required Education, Credential(s) and Experience
  • Education:

    Completion of Licensed Practical Nurse Diploma/Certificate Program or equivalent RN credit hours.
  • Credential(s):
    Basic Life Support (BLS). Licensed Practical Nurse (LPN) and RN/NRP within 4 months of hire, if not already obtained.
  • Experience:

    New graduates may be considered; acute LPN and/or scrub experience preferred.
Preferred Education, Credential(s) and Experience
  • Additional certification:
    Advanced Cardiovascular Life Support (ACLS).
  • Experience:

    2 or more years of LPN experience.
