Your Role:

• Deliver skilled nursing services in-home including wound care, med administration, diabetes care, post-surgical care, injections, and more

• Work 1:1 with patients, building trust and rapport over time

• Collaborate closely with our care team and the patient's physician

• Keep clear, accurate documentation and comply with DOH standards
Who You Are:

• Licensed Practical Nurse (LPN) licensed in Pennsylvania

• CPR-certified and able to travel locally with your own vehicle

• Skilled communicator with a kind, patient-centered approach

• Experience in home health or long-term care is a plus - but not required

• Values relationships, teamwork, and compassionate care
