Licensed Practical Nurse – Home Health

BAYADA Home Health Care is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in Dover, DE. This full‑time or part‑time position serves adult clients on a per‑visit basis in homes throughout Kent County, DE.

One year prior clinical experience as a licensed practical nurse is required. As a home‑care nurse, you will be an integral member of a multi‑disciplinary health‑care team that provides skilled nursing and rehabilitative care, enabling clients to receive the medical care needed to remain at home.

• Make home visits to clients within designated territories.
• Administer medication, wound care, treatments, and procedures as assigned.
• Monitor clients’ conditions and report changes to the Clinical Manager and Client Services Manager.
• Follow up on, execute, and document doctors’ orders accurately.
• Perform client assessments as needed.
• Case manage and coordinate care.
• Accurately document observations, interventions, and evaluations using electronic medical records on touch‑pad tablets.

• A current license as an LPN in the state of DE.
• At least one year of recent, verifiable experience as an LPN.
• Graduation from an accredited and approved nursing program (transcript or diploma).
• Prior home‑care experience preferred.
• Ability to work independently and manage time effectively.
• Strong interpersonal and computer skills; experience with EMR systems preferred.
• Ability to travel to assigned cases.
