As the Case Manager
, you will be responsible for organizing, facilitating, and documenting the service planning for individuals with Developmental Disabilities as well as working collaboratively with other community resources to ensure the quality of service provision.

• Associate degree or equivalent required, Bachelor’s Degree preferred
• Two-three years of relevant field experience preferred
• Excellent oral and written communication skills
• Ability to utilize technology provided to perform requirements of the job
• Hold a valid driver’s license and proof of vehicle liability coverage.
• Must demonstrate excellent multitasking abilities.
