Duly’s Value-Based Care organization is dedicated to transforming healthcare delivery through a patient-centered, outcomes-driven approach. Reporting to the Vice President, Care Management, the Director of Post-Acute & Advanced Care is responsible for providing strategic and operational leadership across post-acute and advanced care programs, including skilled nursing facility (SNF) management, shared decision making, and physician-aligned administrative teams.
This role oversees the development and execution of value-based care initiatives that drive quality, utilization, and cost performance across the post-acute continuum. The Director partners closely with SNF and home health vendors, hospitalists, geriatricians, and internal care management teams to establish clear workflows, coverage models, and performance expectations. The position also ensures adherence to health plan requirements, regulatory standards, and contractual obligations while advancing best-in-class patient outcomes and continuity of care.
ResponsibilitiesPerformance, Outcomes and KPIs
- Provide strategic and operational oversight to drive clinical, quality, utilization, and cost outcomes aligned with Value-Based Care objectives.
- Define, monitor, and manage program-specific KPIs across post-acute and advanced care services, including SNF performance, transitions, and shared decision making.
- Partner with analytics and enterprise teams to define reporting requirements and ensure accurate, actionable dashboards to support performance management.
- Lead regular performance reviews with SNF partners and vendors to assess outcomes, identify trends, and implement improvement plans.
- Establish and enforce performance expectations for post-acute partners, including accountability for avoidable admissions, length of stay, and quality outcomes.
- Ensure Value-Based Care patients are appropriately identified, attributed, and supported through Duly providers and care management teams.
- Provide day-to-day leadership and strategic direction across post-acute and advanced care programs to ensure alignment with organizational goals.
- Oversee post-acute network staff and physician‑aligned administrative teams, including hiring, onboarding, training, and ongoing development.
- Establish clear structure, coverage models, workflows, and expectations for administrative teams supporting hospitalist and geriatrician physicians.
- Lead performance management activities, including coaching, feedback, evaluations, and career development, while addressing employee relations matters as needed.
- Meet regularly with direct reports to review performance, operational challenges, and opportunities for improvement.
- Develop and maintain educational resources, tools, and workflows to support staff effectiveness and program consistency.
- Oversee patient advocacy and issue resolution processes to ensure timely, appropriate response to patient and partner concerns.
- Collaborate with leadership to determine and achieve performance targets.
- Collaborate with the VP, Care Management to develop and operationalize strategic plans for the post‑acute and advanced care programs.
- Serve as a liaison between Duly and its key stakeholders to promote, improve and communicate initiatives related to Value-Based Care Programs.
