Job Summary

The Hospice RN Team Leader oversees patient care delivery, coordinates interdisciplinary teams, and ensures compliance with care plans and standards. Manages nursing schedules, supports admissions and discharges, and acts as a liaison between patients, families, and the care team to provide high‑quality, compassionate care.

Qualifications

Education: Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.

Licensure: Current state license as a registered nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. Current driver’s license.

Experience: Minimum of two years’ experience as a registered nurse in a clinical care setting. Hospice experience required. Management experience preferred.

Skills: Nursing skills as defined as generally accepted standards of practice. In‑depth knowledge of hospice regulations, Medicare/Medicaid guidelines, and best practices in end‑of‑life care. Strong leadership, communication, and problem‑solving skills. Compassionate, empathetic, and committed to the mission of hospice care. Must read, write, and comprehend English.

Transportation: Reliable transportation and valid auto liability insurance.

Environmental And Working Conditions

Performs duties in an office environment during operating hours. If required to make patient visits, may work in patients’ homes or facility in various conditions; possible exposure to blood and bodily fluids and to infectious diseases; ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency calls. Must be available to work on call.

Essential Functions
  • Oversee team operations, including triaging referrals, patient calls, and resource allocation.
  • Provide clinical leadership for clinical staff, assist with rostering, budgeting, and daily scheduling.
  • Coordinate interdisciplinary team meetings, chart reviews, and follow‑ups.
  • Ensure compliance with hospice protocols, develop policies, and deliver education to staff and community partners.
  • Evaluate and manage patient admissions, discharges, and hospice eligibility criteria.
  • Utilize the nursing process to provide home‑based care and maintain thorough documentation.
  • Conduct comprehensive assessments addressing physical, psychosocial, and environmental needs.
  • Develop, implement, and update care plans to ensure quality and continuity of care.
  • Educate patients, families, and caregivers on disease management, self‑care, and end‑of‑life care.
  • Provide clinical direction to clinical staff, ensuring adherence to care standards.
  • Maintain detailed progress notes, communicate with physicians, and monitor third‑party compliance.
  • Perform on‑call responsibilities and ensure effective communication with patients, families, and healthcare teams.
  • Assess and document patient needs and care plans in coordination with the interdisciplinary team.
  • Provide nursing care adhering to infection control protocols and ensure timely paperwork submission.
