We are seeking a Licensed Vocational Nurse (LVN) to provide high‑quality nursing care in patients’ homes within the Woodland Hills/SF Valley area. The LVN will plan and deliver care following company, federal, state, and local standards to ensure dignity and quality for each resident.

• Plan and deliver nursing services in accordance with all applicable standards, guidelines, and regulations.
• Aid and educate families within a hospice care setting.
• Drive to patients’ homes throughout the Woodland Hills and surrounding areas.

• Nursing degree from an accredited college or graduation from an approved LVN program.
• Unencumbered license to practice as an LVN in California.
• Proficiency with electronic medical equipment.
• Knowledge of nursing/medical practices, procedures, terminology, laws, regulations, and guidelines relevant to home health.
• Experience in a home‑health setting.
• Own vehicle available for patient visits.
• Current driver’s license and proof of insurance.
• On‑call availability.

• Direct care experience with multiple patients.
• Experience in a hospice environment.
