Unit Description – Case Management Department
Type of Unit:
• Dedicated Case Management and Social Services department overseeing patient care coordination, discharge planning, and utilization review across all inpatient and outpatient areas.
Primary Focus:
• Ensures continuity of care through interdisciplinary collaboration and effective transition planning from hospital to home or post-acute settings.
• Manages patient throughput while balancing quality outcomes, patient satisfaction, and cost-effective care.
Shift Details:
• 8-hour day shifts: 08:30 AM – 05:00 PM
•
Schedule:
Monday–Friday, with weekend or on-call rotation based on department needs.
Core Responsibilities:
• Perform comprehensive assessments of patients’ discharge needs and coordinate individualized plans of care.
• Conduct concurrent and retrospective reviews to ensure medical necessity and appropriate utilization of services.
• Collaborate with physicians, nursing staff, and insurance providers to expedite care transitions.
• Facilitate DME, SNF, rehabilitation, home health, and transportation arrangements.
• Participate in interdisciplinary rounds to evaluate progress and barriers to discharge.
• Manage appeals, authorizations, and denials in compliance with payer requirements.
• Document all case management activities accurately in Allscripts/Care Port.
Required Experience &
Competencies:
• Minimum 2 years of acute care case management or discharge planning experience.
• Strong knowledge of Inter Qual and Milliman criteria for level-of-care determination.
• Proficiency in CMS regulations, HIPAA compliance, and Joint Commission standards.
• Demonstrated ability to coordinate complex discharges and collaborate effectively across departments.

Pyxis (for coordination with clinical teams)
Patient Population:
• Adults and geriatrics across all inpatient service lines, including medical, surgical, and critical care units.
Certifications:

May be required across multiple service areas within scope.
