Responsibilities
- Review patient's drug regimen on a concurrent basis and communicate any suggested changes to the prescribing physician. Verify the physician's original order for appropriateness prior to dispensing
- Provide patient education, counselling regarding drug therapy, drug related disease prevention and proper use of medication.
- Prevention, detection and resolution of a drug related problem.
- Process electronic records of medications dispensed to hospitalized patients. Fill, check and dispense outpatient and inpatient prescriptions issued by physicians
- Prepare, package, dispense and deliver unit dose medication in accordance with standard procedures and techniques.
- Maintain and control of the floor stocks of drugs kept in the wards and nursing units.
- Prepare drug formulations, strength, dosage forms, and packages that are useful in the care of patient and not available commercially but approved by the Pharmacy and Therapeutics committee.
- Prepare all sterile products (e.g. Chemotherapy injections, IV admixture, TPN}).
- Perform routine inspection of all drug storage area within the hospital in order to ensure that no outdated or unusable drugs are present.
- Perform any other tasks as assigned by line manager
- Bachelors degree with 3 years of experience OR
- Masters degree with 1 year of experience
