Registered Nurse - Home Care IV Cannulation

Job Summary

Conduct comprehensive in-home patient assessments, including vital signs, physical examinations, and medication reviews, to establish a baseline and identify changing health conditions. Develop and implement individualized care plans tailored to each patient’s specific needs and goals, ensuring alignment with physician orders and patient preferences. Administer medications—including oral, intravenous, and subcutaneous injections—and manage central lines, ports, and other specialized medical equipment.

• Ensure alignment with physician orders and patient preferences.
• Administer medications, including oral, intravenous, and subcutaneous injections.
• Manage central lines, ports, and other specialized medical equipment.

• Minimum of 2 years of experience in a clinical setting, preferably with experience in home health or a related field.
• Exhibits excellent communication and interpersonal skills, with the ability to build rapport with patients and families.
• Must have the ability to work independently and make sound clinical judgments.
