Responsibilities
Conduct comprehensive health assessments to create personalized care plans that address the unique needs of each client.
Administer medications and treatments as prescribed, ensuring adherence to the specific medical protocols and safety standards.
Monitor vital signs and health changes, documenting observations meticulously to inform ongoing care decisions.
Provide emotional support and education to clients and their families, empowering them to manage their health conditions effectively.
QualificationsBachelor's degree in Nursing (BSN) or equivalent; a higher qualification is preferred for advanced roles.
Active nursing license in the relevant jurisdiction, with a preference for candidates holding additional certifications in home care.
Minimum of 2 years of nursing experience, with at least 1 year specifically in home care or community health settings.
Experience with geriatric care, chronic disease management, and palliative care is highly desirable.
