Base pay range$14.00/hr - $14.00/hr
LocationBleckley County, GA (THIS IS NOT A REMOTE POSITION)
Responsibilities of the Apex CSI Case Manager- Providing a broad range of social services to individuals served in Emanuel County Schools and their families
- Ensuring individuals go to their scheduled doctor appointments and take medication as prescribed
- Contacts consumers parents, teachers, and school counselors on a weekly basis and reporting findings to supervisor and MD, ensuring all progress notes are correct
- Completing progress notes, no later than 24 hours after delivering the service, having said progress notes scanned and shown in the EMR no later than 7 days after providing the services, and checking EMR weekly to ensure that progress notes are showing in EMR
- Working with scheduler in completing weekly schedule to ensure monthly contacts are met and maintained
- Communicating regularly with staff on progress toward defined goals and/or required results, providing specific feedback, and initiating corrective action when defined goals and/or required results are not met
- All other duties as assigned
- High School Diploma/GED Equivalent
- Valid Georgia Driver’s License
- Effective verbal and written communication skills
- Strong interpersonal skills and the ability to work effectively with diverse communities
- Ability to work independently and in collaboration with others
- Experience with Microsoft 365 Office Products
- Ability to organize, prioritize and meet deadlines accordingly
- Bachelor’s degree from an accredited college or university in the field of psychology, social work, counseling education, human services or a related field of study
- Previous case management experience
