- Provide 1:1 care for clients following the client's plan of care at all times while maintaining client's dignity and independence
- Administer medications, feedings, oxygen, ostomy care as needed per client's plan of care and provide assessments of results
- Accurately document nursing actions and communicate with family, case manager, and physicians, documenting progress and outcomes for established goals and informing stakeholders of changes in client's medical condition and needs
- Coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are always on hand
- Ensure all patient needs are being met in accordance with the instructions of the physician
- LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
- Valid driver’s license, acceptable driving record and proof of car insurance
- CPR professional certification
- 1+ years of nursing experience preferred
- Home Health care experience preferred

Adoration Home Health / Bright Spring Health Services, an affiliated company, will initially employ the successful candidate through LHC Group until January 1, 2026. During this period, LHC Group will perform all administrative onboarding activities and offer benefits coverage under LHC Group employee benefit plans. On or before January 1, 2026, the successful candidate’s employment will be transferred to Adoration Home Health / Bright Spring Health Services.
At all times, Adoration Home Health / Bright Spring Health Services will make all hiring decisions and supervise the work of the successful candidate.
