Title: Occupational Therapist (OT) – Home Health / Hospice
Location: Dublin, OH
Duration: 13 weeks
Shift: Day shift, 5x8 hours (8:00 AM – 5:00 PM)
Seeking an experienced Occupational Therapist for Home Health/Hospice care. The role involves managing a caseload of geriatric patients within a defined territory and providing high-quality, timely therapy services. Candidates must be comfortable working independently, traveling within the assigned area, and maintaining accurate documentation.
Job ResponsibilitiesProvide occupational therapy services to geriatric patients in a home health/hospice setting
Manage an average caseload of 5–7 patients per day
Complete Start of Care (SOC) documentation when required
Ensure timely, accurate documentation and clear communication
Collaborate with interdisciplinary care teams
Participate in on-call rotation (approximately every 6th weekend)
Travel within a territory up to a 60-mile radius
Comply with dress code and professional standards
Skills & Qualifications
Minimum 1 year of recent OT experience
Home Health and/or Hospice experience required
OASIS experience required for SOC visits
Proficiency with HCHB (Homecare Homebase) EMR required
BLS certification required
Active OT license in hand at submission
Valid driver's license and reliable personal transportation
Ability to be flexible with territory assignments
Strong documentation and communication skills
NO first-time travelers
