• Provide direct care for all assigned patients including initiation and maintenance of all communications with care team members.
• Evaluate and educate patients/care partners to safely administer home infusion therapy.
• Evaluate home environment and identify necessary interventions to ensure safety.
• Develop and maintain plan of care and evaluate response to therapy with each encounter.
• Maintain caseload scheduling via company approved platform and communicate scheduling changes.
• Coordinate care via approved company platform.
• Takes part in nurse on-call rotation to ensure continuous support services.
• Document all communications at time of service per policy.
• Strong knowledge of therapies provided by Soleo.
• Work with departments to ensure required patient documents are completed and returned.
• Ensure timely submission of plan of treatments as required by state guideline.

• Part-time: 4 days per week, Monday-Friday 12pm-8pm and 1 weekend day – 30-34 hours per week, no holidays.
• Knowledge of IV access devices including IV lines, ports, and infusion medication administration.
• Participation in on-call rotation: 1 weekday every 2 weeks and 1 weekend every 2-3 months.
• Drive up to 2 hours one direction to see patients in homes as needed.
• Work in patients’ homes around animals and smoke.
• Hold a single state RN license for Ohio.

• RN Degree, BSN Preferred.
• Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required.
• Minimum 3 years nursing experience with background in infusion therapy preferred.
• Current nursing license for Ohio.
• Experience in ICU and/or Cardiac Care preferred.
• Proficient with Microsoft Outlook and Microsoft Office Suite.
• Valid state-issued driver’s license with no significant restrictions.
